2. Please download and fill out Carpe Diem Arts Medical Form and Waiver. Once signed, send to email@example.com.
3. Pay via Paypal or check made out to Carpe Diem Arts, sent to 425 Greenbrier Dr., Silver Spring, MD 20910. Note: In order for us to reserve a space, we ask that you pay a deposit of 50% of tuition at the time of registration (French and Spanish Immersion camps: a deposit total of $195; Sleepaway camp: a deposit total of $437.50). The tuition balance is due June 1st. We welcome full payment before June 1, if possible. Please see pricing chart below:

*CANCELLATION POLICY: If cancellation is made before May 1st, we can offer a full refund, minus $75. If cancellation is made at least one month prior to the start date of camp, we can offer a 50% tuition refund. We cannot give any refunds within one month of camp’s start.
